Athletes under age 18 must have signed parental consent. Access the parental consent forms here.
Walk in or schedule an appointment for Tuesday, July 7th only. Sports physicals are free of charge, but we request a donation of 2 unopened hygiene products OR a $10 monetary donation to give back to our local schools.
Physicals are on a first come, first served basis with no appointment needed. However, you may schedule an appointment by calling 715-359-6442.
